Group Participation Waiver

A Hospice Yukon group is intended to provide gentle grief support. 

If you are increasing your activity level or will be doing physical activity your body is unaccustomed to, please seek the guidance of a medical professional before joining this group. 

I authorize Hospice Yukon staff and volunteers to provide first aid treatment to me and/or contact Emergency Medical Services if in their reasonable discretion they determine such services are needed.

I acknowledge that any activity involves risk.  I freely assume those risks as a requirement of my participation in activities facilitated by Hospice Yukon.
